Becoming a Patient's Support Person

Trans Solidarity in Partnership with PULSE of NY, which is a community based patient safety organization available to help patients and their family receive the support they need when receiving medical care. If you are willing to be a support person, we will share that information with patients who may need you.

How it works:

The goal of Family Centered Patient Advocacy is to establish a support network for the patient with predetermined roles and tasks for all participants. The patient advocate or care coordinator (CC) , from PULSE of NY or someone else acting in this role serves as a mediator between the patient and their family and friends. When someone is being hospitalized or in need of extensive medical treatment, and contact PULSE of NY, they will be asked for a list of people they would feel comfortable accepting assistance from. If your name is on their list, you will be contacted. If you know someone who may need help, you can complete the attached questionnaire ahead of time so we know you are interested.

Organizing a Support Person or Support Team; Patient Information

Trans-Solidarity partnering with PULSE of NY, which is a community based patient safety organization available to help patients and their family receive the support they need when receiving medical care. If you are in need of a support person, we will do our best to help match you up with people willing to help.

How it works:

The goal of Family Centered Patient Advocacy is to establish a support network for the patient with predetermined roles and tasks for all participants. The patient advocate or care coordinator (CC) , from PULSE of NY or someone else acting in this role serves as a mediator between the patient and their family and friends. When you are being hospitalized, you will be asked for a list of people you would feel comfortable accepting assistance from. You can also provide a list of people who you would prefer not to be involved. If someone offers to assist and they are not on your approved list, you will be notified by the care coordinator. If you do not want someone to be involved, the care coordinator will simply tell them that you already have enough help.

Together, you and the care coordinator will decide what your needs will be both in the hospital and at home. The care coordinator will then approach the people on your approved list with suggestions for how they can help you. This allows them the opportunity to say “no” when they do not feel comfortable or feel that they are not capable of providing certain assistance. The care coordinator, as a third party allows them to say “no” to a request without feeling guilty. If they are eager to help in another way, the care coordinator can work with them to figure out an alternative.

The care coordinator will approach the people on the “yes” list and ask them how they can help. Can they go to the hospital? Can they cook, clean, or drive? It is best to choose people who you would be willing to help if they were in need.

After the care coordinator has spoken with people on your “yes” list, he /she will go back to you with the information acquired. Together you will review who has volunteered to assist you and what they will help you with. If you are not satisfied, changes can be made without revealing your reasons to them.

It has been our experience that the presence of a neutral, third party individual encourages more people to step up and be involved.
